As you can see in the cross-sectional view of the hub-centered wheel spacer above, the retaining ring fits into the space on the wheel exactly as the hub's retaining ring does. This means that the weight of the car still rests on the hub rather than on the wheel bolts, which means the wheels don't vibrate and also protects the bolts.

However, with non-hub center wheel spacers, the weight of the car is no longer on the retaining ring, but on the studs, making it easier to cause wheel vibration, and the studs carry all the weight of the car. That doesn't mean you can't go with off-hub center wheel spacers, just make sure the studs are good quality.
